Reference is made to the announcement jointly issued by Sino Wealth International Limited and Clear Prosper Global Limited dated 16 February 2026 in relation to, among other things, the payments to the Relevant Shareholders (the “Announcement”). You should read the following forms in conjunction with the Announcement. Unless otherwise defined, capitalised terms used herein shall have the same meanings as those defined in the Announcement.

Claim Form
If you wish to make a claim, you are required to complete the following claim form and submit, together with the relevant documents, to the Adjudicator no later than 5:00 p.m. on Saturday, 16 May 2026.
Supplemental Claim Form
If your claim is unsuccessful, you may complete and submit the following supplemental claim form, together with the relevant documents, if any, to the Adjudicator no later than 5:00 p.m. on Sunday, 16 August 2026.
